What does private health insurance cost in Germany in 2026?

Private health insurance premiums are based on your age, health, and chosen tariff when you join, not on your income. A healthy professional in their early 30s typically pays between €380 and €550 per month for solid mid-tier cover, before the employer share is deducted. In 2026, employers contribute up to €508.59 per month toward health insurance plus €104.63 toward long-term care, the same maximum subsidy public members receive.

Public health insurance, by contrast, costs around 17.5 percent of gross salary including the average additional contribution, plus long-term care insurance. Contributions are calculated on your income up to the contribution assessment ceiling (Beitragsbemessungsgrenze, €5,925 per month in 2026), where the employee share comes to around €650 per month including long-term care. That is why many high earners pay less in private cover at entry, often with broader benefits.

Access is regulated. Employees can choose private health insurance once their gross salary exceeds €77,400 per year (the 2026 threshold, adjusted annually).   • Getting older won't affect your price

    The price is based on your age when you sign up. It then only increases when overall healthcare costs or inflation go up, historically between 0% and 2% per year on carefully selected tariffs.

How do you switch from public to private health insurance?

Done in the right order, the switch is straightforward. Step one is the health questionnaire and a binding acceptance from the private insurer, with your target tariff locked in. Only then do you give notice to your Krankenkasse, which normally takes two months to the end of the month. Your new policy starts the day public cover ends, so there is no gap.

Timing matters more than most people expect. Employees need to have cleared the income threshold, and a switch around a salary raise or a job change follows special rules, as does January 1. Get the sequence wrong and you can end up paying double, or stuck as a voluntary public member for another year.
